Office Policies and Client Responsibilities
A. Insurance Information: You will need to call your insurance company before
your first office visit to get pre-authorization
for your sessions. The insurance company will give you an
authorization number and the number of office visits allowed.
Failure to contact your insurance company for this information prior to
your visit would likely result in non-payment from the insurance
company for the visit. Be sure to bring your insurance card to
your first session.

Insurance Information Needed: Ask the insurance
company what your co-payment will be for each visit and find out if you
have a deductible to meet before insurance will cover your visits.
B. Cancellations: When it becomes necessary to reschedule or cancel your appointment, a 24 hour notice is required to avoid being charged for that visit. You will be automatically billed the entire fee for late cancellations or missed appointments, therefore we cannot just accept the co-payments for these sessions.
Late cancellations and missed appointments are frustrating to all therapists. It often prevents others, who may have needed that time, from being seen and certainly is lost income for the therapist.
C. Payment: Payment/co-payment is expected at the time of your visit unless you have coverage through an Employee Assistance Program or insurance that has been determined to provide coverage. If insurance does not cover any or all of the session fee, you are responsible for any uncovered portion.
D. Session Time: Your sessions will be 50 minutes long . Typically, the session will start at the top of the hour and go to 10 minutes before the hour.
